Dear UHS Families,
We hope this letter finds you and your family faring well and we are looking forward to supporting you throughout this unique school year. In this letter, there are two important action items that require your attention. Thank you for taking the time to review and respond.
It is our pleasure to introduce you to One Medical, an online membership-based primary care platform, with whom we will be partnering this year. We will be using One Medical’s screening app for all students and employees for entry clearance. Although students will not be coming to campus during the first quarter, we will be asking students and families to download the app this week, familiarize yourselves with the screening process, and ultimately to use it in the coming weeks so by the time we open our campus, all of us are familiar and comfortable with the screening process and the use of the app. More information will be coming through the UHS administration when we begin to formally use the app for students.
In addition to the screening app for UHS students, One Medical provides online primary care for every member of your immediate family. We will be using One Medical’s testing services, so it is essential that at least every UHS student is signed up for One Medical. Below is the link to sign up for One Medical (using Chrome of Firefox browsers).

Please note, you do not have to enter your credit card information when signing up, but rather scroll further down and you will see the place to enter the above CODE. This CODE can also be used to sign up immediate family members, so parents and siblings are also covered under the One Medical plan.
Once you have signed up at One Medical you will be able to download the app.
We encourage you to sign up today. One Medical will be following up with you if you haven’t registered this week.
One Medical is available to help with any questions around signing up, as well as to discuss the specifics of what is offered under the plan and how it works.
Also, we ask that you review with your family our Pledge of Mutual Responsibility and sign by September 8. We recognize that while we remain in Level 3 (campus closed), the sooner we compile documentation, make screening a habit, and discuss and practice healthy habits, the better prepared our community will be to open our campus when conditions permit.
